Moodle: Questionnaire

Overview

The Questionnaire activity on Moodle allows the creation of a set of questions. The activity is designed for gathering responses from users within a Moodle course. The Questionnaire is used for gathering data, rather than as an assessment tool: it doesn’t contain grading options. 

Adding a Questionnaire in Moodle 

  • Switch Edit mode on 
  • Click Add activity or resource 
  • Select Questionnaire

  • Add a Name and optional Description 
  • Under Availability set dates during which Questionnaire is accessible 
  • Under Response options: 
    • Use Type to change the number and frequency of responses allowed 
    • Under Respondent type anonymity can be added 
    • Determine whether Students can view ALL responses 
    • Set Submission notifications 
    • Set whether partial responses can be saved 
    • Choose whether to use Branching questions 
    • Finally set question numbering options and add an optional grade for completing (note this is for the questionnaire as a whole, rather than per question) 
  • Under Content options, choose whether to re-use an existing questionnaire, or select Create new 
  • Click Save and display

Configuring the Questionnaire 

Click Add questions

Under Add questions, select a Question type.

Complete details for the selected question type. These will vary depending on selection, as a minimum questions will require the Question text box to be completed.

Click Save changes

The Manage questions section allows you to manage question order, as well as update or delete questions.

Click the Preview tab for a view of how your Questionnaire will appear 

Advanced settings includes options to re-direct users of the Questionnaire to a web page or new Moodle page once completed. Email notifications for completed responses can also be configured here.

The View all responses tab will appear once the first response to your questionnaire has been completed. This provides an overview of question responses, plus more detailed breakdown of individual questions or users. Click a respondent’s name for a single-response view.

Non-respondents will show all users within a Moodle course yet to complete the Questionnaire 

To return to this window after closing the questionnaire, click Open course menu, then Questions

Managing the Questionnaire 

To modify a question: 

  • Click Open course menu 
  • Select Questions tab 
  • Open the Manage questions section 
  • Click the Edit cog at the top of the question box

Modifying questions with responses will retain any existing answers. To create an unanswered question, use the Save as New Question option to create an updated, unanswered question.  

To delete a question: 

  • Click Open course menu 
  • Select the Questions tab 
  • Open the Manage questions section 
  • Click the Delete X at the top of the question box

To delete an individual response:

  • Click Open course menu 
  • Select the More tab, then click View all responses 
  • Select List of responses, then click the user from the list of respondents 
  • Choose Delete this response
  • Click Delete in the confirmation window

To delete all responses: 

  • Click Open course menu 
  • Select the More tab, then click View all responses 
  • Click Delete ALL responses 
  • Choose Delete in the confirmation window

To download a report of all responses: 

  • Click Open course menu 
  • Select the More tab, then click View all responses 
  • Click Download 
  • Choose relevant options and the report type 
  • Click Download
